The ultimate nuclear endgame.
Nuclear weapon alone is no longer good enough as a deterrence, in a missile war conflict, the size of a country can matter.

Once a nuclear war is started, all countries with nuclear will be fair game. Blow for blow, smaller size countries would get wipe out first.

(Side note: fact of the matter is - it is better off for small size countires not possessed any nuclear weapon. AIMHO.)

In a large scale nuclear war, only countries or people living far enough away from nuclear hit cities will have a smallest chance of survival.
Their long-term survival depends on access to food, water, medical supplies, and the ability to cope with potential societal disruption in freezing temperature.
